Job Description

Join a growing team delivering patient-centered primary care in skilled nursing facilities across Kenosha, Wisconsin. This full-time opportunity offers a flexible schedule with no overnight or on-call responsibilities and the chance to collaborate closely with physicians, nursing staff, and interdisciplinary teams to provide high-quality, evidence-based care.

Position highlights

  • Full- and part-time schedules with flexible six-hour facility shifts; rounds generally begin between 7:00 a.m. and 10:00 a.m.
  • No call or overnight shifts
  • Typical caseload of 15-20 patient visits per day
  • Conduct daily rounds, manage acute and chronic conditions, prescribe medications, and develop individualized treatment plans
  • Work collaboratively with nursing staff and interdisciplinary teams to ensure coordinated care
  • Maintain compliance with clinical and regulatory standards
  • Company-provided iPad with LTE and professional lab coat

Requirements

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • NP board certification (ANCC, AANPCB, PNCB, or AACN) or PA certification (NCCPA)
  • Board certified or eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine preferred
  • Strong clinical judgment, communication, and patient-care skills
  • Ability to work independently while collaborating effectively within a multidisciplinary team
