Hospitalist Nurse Practitioner / Physician Assistant - Skilled Nursing (Bonham, TX)

Join a team devoted to delivering compassionate, patient-centered care to residents of skilled nursing facilities across Bonham, Texas. We’re looking for dedicated Nurse Practitioners and Physician Assistants who enjoy working with older adults and thrive in a collaborative, team-based setting.

What you’ll do

• Provide daily rounds in skilled nursing facilities, managing acute and chronic conditions and developing treatment plans
• Prescribe medications and coordinate care with facility staff and families
• Work efficiently using our streamlined EHR with AI scribe, voice dictation, and a company iPad with LTE
• Use clinical resources like UpToDate® for real-time guidance

Schedule and work environment

• Full-time positions with flexible scheduling
• Six-hour facility shifts for full-time providers (rounds typically start between 7:00 a.m. and 10:00 a.m.)
• No overnight or on-call responsibilities

What we’re looking for

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
