Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing Facilities (IL)

We seek compassionate Nurse Practitioners and Physician Assistants who are committed to improving the health and quality of life of older adults in a collaborative, team-based setting.

What you’ll do

• Provide daily on-site rounds in skilled nursing facilities, managing acute and chronic conditions and implementing treatment and medication plans
• Work six-hour facility shifts for full-time providers (rounds typically start between 7:00 a.m. and 10:00 a.m.)
• No overnight shifts or on-call duties
• Document efficiently using a streamlined EHR with AI scribe, voice dictation, and a company-issued iPad with LTE
• Access UpToDate® for real-time clinical guidance
• New graduates encouraged to apply

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license in Illinois
• Board certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
