Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing (Pratt, KS)

Join a growing team providing patient-centered care to residents in skilled nursing facilities in Pratt, Kansas. We are hiring Nurse Practitioners and Physician Assistants to manage acute and chronic conditions in a team-based environment.

Key responsibilities

• Perform daily rounds in skilled nursing facilities
• Assess and manage acute and chronic illnesses; initiate and adjust treatment plans and medications
• Coordinate care with facility staff, families, and consulting clinicians
• Complete timely documentation in the EHR using AI Scribe and voice dictation
• Use UpToDate® and other clinical resources for point-of-care decision support

Schedule

• Part-time and full-time opportunities
• Full-time: six-hour facility shifts; rounds typically begin between 7:00 a.m. and 10:00 a.m.
• No overnight shifts or on-call responsibilities

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• Board certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent)
• Preferred: experience in skilled nursing, long-term care, geriatrics, or post-acute care
