Family Medicine Nurse Practitioner - Value-Based Primary Care (Tacoma, WA)

Join a primary care team delivering evidence-based, patient-centered care within a medical home model. The NP will partner with patients and interdisciplinary team members to develop biopsychosocial care plans, provide clinical leadership, and manage chronic and acute conditions.

Key responsibilities

• Provide primary care to 10-15 patients per day: prevention, chronic disease management, acute care, and case management.
• Develop individualized care plans with patients and the care team, including an onsite Whole Health Coach, respecting diverse backgrounds and needs.
• Perform comprehensive health histories, physical exams, diagnostic testing, and prescribing; utilize onsite lab and pharmacy.
• Drive population health initiatives: assess patient needs, promote annual whole health evaluations, health coaching, and preventive activities.
• Provide clinical leadership: participate in team meetings, communicate proactively, prioritize initiatives, support process improvement and change management.
• Rotate with regional providers for after-hours coverage.

Compensation & benefits

• Base salary: $153,000-$178,000.

Requirements

• NP or PA board certification in primary care, family medicine, or internal medicine.
• Current, unrestricted ARNP/PA license and valid DEA number.
• 3-5 years primary care experience.
• Minimum 2 years case management experience with emphasis on prevention and population health management.
• Experience with EMR systems (NextGen preferred).
• Coding and documentation experience, including Medicare Advantage and HCC.
