Family Medicine Floating Nurse Practitioner or Physician Assistant - Laurens, SC (Multi-Site)

Seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to serve as a floating family medicine provider supporting multiple outpatient clinics across Abbeville, Greenwood, Laurens, McCormick, and Saluda counties, SC. The provider will deliver patient-centered primary care with a focus on prevention and chronic disease management.

Primary responsibilities

• Provide comprehensive primary care across the lifespan (wellness, acute and chronic care).
• Travel regularly between assigned family medicine sites within Abbeville, Greenwood, Laurens, McCormick, and Saluda counties.
• Deliver evidence-based care and coordinate treatment plans with physicians, nurses, and other team members.
• Educate patients and families about diagnoses, medications, self-care, and preventive health.
• Accurately document patient encounters in the electronic health record in a timely manner.

Minimum qualifications

• Graduate of an accredited NP (APRN) or PA program.
• Minimum 2 years’ experience in primary care or family medicine.
• Active, unrestricted South Carolina license.
• Certified or eligible for certification in specialty (e.g., FNP, PA-C).
