Floating Family Medicine Nurse Practitioner or Physician Assistant - Abbeville, SC (Multi-Site)

We are hiring a Nurse Practitioner or Physician Assistant to serve as a floating family medicine provider across outpatient clinics in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties, SC. The provider will deliver primary care services across the lifespan with emphasis on prevention, wellness, and chronic disease management.

Responsibilities

• Provide comprehensive primary care for patients of all ages, including assessment, diagnosis, treatment, chronic disease management, and preventive care.
• Travel regularly to assigned family medicine sites within Abbeville, Greenwood, Laurens, McCormick, and Saluda counties.
• Deliver evidence-based care and patient education about treatment plans, medications, and health maintenance.
• Coordinate care with physicians, nurses, and other clinical staff.
• Accurately and promptly document patient encounters in the electronic health record.
• Maintain clinical standards, comply with policies, and participate in quality improvement activities.

Requirements

• Graduate of an accredited NP or PA program.
• Minimum 2 years’ experience in primary care or family medicine.
• Active, unrestricted South Carolina medical license.
• Certified or eligible for certification (FNP or PA-C).
• Reliable transportation and ability to travel between clinic sites.
