Float Family Medicine Nurse Practitioner (FNP) - Phoenix, AZ

Join our West Phoenix, AZ team as a full-time Family Nurse Practitioner providing flexible, high-quality primary care across multiple clinic settings. In this float role you will deliver patient-centered assessments and treatment, manage common acute and chronic conditions, coordinate care with interdisciplinary teams, and provide education to patients and families to support health and prevention.

Key responsibilities

• Provide comprehensive primary care across diverse clinical environments, including acute visits, chronic disease management, and well-care.
• Perform history and physical exams, order and interpret diagnostic tests, develop and implement treatment plans, and prescribe medications as appropriate.
• Facilitate care continuity through collaboration and effective handoffs with physicians, nurses, and ancillary staff.
• Educate patients and caregivers on diagnoses, medications, self-management, and prevention.
• Adapt quickly to different clinic workflows and populations while maintaining high-quality, efficient care.

Qualifications

• Master’s or Doctoral degree in Nursing with Family Nurse Practitioner focus.
• Current FNP certification and active Arizona RN/APRN license.
• DEA registration.
• Clinical experience in family medicine or primary care preferred.
• Excellent communication, interpersonal skills, and the ability to thrive in varied clinical settings.
