What you’ll do

• Provide comprehensive primary care for patients across the lifespan: assess, diagnose, and manage acute and chronic conditions.
• Promote health and prevention through patient education, screening, immunizations, and wellness counseling.
• Coordinate care with physicians, behavioral health providers, social workers, and support staff to create and follow individualized care plans.
• Prescribe and manage medications, monitor responses, and identify potential side effects.
• Perform clinical procedures such as suturing, wound care, minor interventions, and routine immunizations.
• Maintain accurate, timely electronic health records and comply with regulatory documentation standards.
• Participate in quality improvement activities and help refine clinical protocols and workflows.
• Stay current with best practices through continuing education and professional development.
