Key responsibilities

• Deliver evidence-based assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions.
• Create and maintain accurate, compliant medical records.
• Coordinate care with interdisciplinary team members and community resources.
• Participate in quality improvement and patient education initiatives.

Qualifications

• Graduate of an accredited medical school.
• Completion of an accredited U.S. Family Medicine residency.
• Board certified in Family Medicine.
• Active Arizona medical license and DEA certificate.
• Current BLS certification.
• Minimum three years’ community health experience.
• Bilingual English/Spanish preferred.
• Cultural competence and strong interpersonal skills.
• Proficiency with basic clinical methods and standard office computer tasks.
• Basic business English, spelling, and punctuation skills.
• Valid Arizona driver’s license.
