Required Credentials

• Active, valid Physical Therapy state license.
• CPR and BLS certification from American Heart Association (AHA) or Red Cross (must include AED).
• Photo identification (state or federal) to present at facility; registry personnel must wear facility photo badge at all times.

Core Responsibilities

• Perform comprehensive physical evaluations and establish individualized treatment plans.
• Design and implement exercise programs to improve strength, flexibility, endurance, and coordination.
• Deliver hands-on interventions to improve mobility and reduce pain.
• Apply therapeutic modalities and instruct patients in use of assistive devices as appropriate.
• Provide patient and family education and maintain accurate, timely documentation of progress and treatments.
