Job Description

We’re expanding our team that delivers patient-focused care to residents in skilled nursing facilities around DeKalb, IL. If you’re a compassionate Nurse Practitioner who enjoys working with older adults in a collaborative, team-based setting, we’d love to hear from you. Opportunities also exist in MA, MI, NH, NC, OH, and WI.

What you’ll be doing

  • Provide daily rounds at skilled nursing facilities, managing both acute and chronic conditions.
  • Develop and adjust treatment plans, prescribe medications, and coordinate care with facility staff and families.
  • Work efficient shifts (typically six hours) with rounds beginning between about 7:00 a.m. and 10:00 a.m.
  • Document care using a streamlined EHR enhanced by AI Scribe and voice dictation on a company-provided iPad with LTE.
  • Use UpToDate® for current, evidence-based clinical guidance.

Who we’re looking for

  • Active, unrestricted Nurse Practitioner license and current board certification (ANCC, AANPCB, PNCB, AACN, or equivalent).
  • Prior experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred.
  • Compassion for older adults, strong communication skills, and a team-oriented approach to care.
