Job Description

Provide patient-centered primary care to residents in skilled nursing facilities across Coulterville, IL. This part-time role offers flexible scheduling, no nights or on-call, and collaborative work with physicians and facility staff to deliver evidence-based care.

Locations

  • Primary: Coulterville, IL
  • Additional IL facilities: Flanagan, Galesburg, Henry, Ridgway
  • Nationwide opportunities: GA, IN, KS, MI, MO, OH, TN, WI

Schedule & Workload

  • Part-time with flexible scheduling
  • Six-hour facility shifts; rounds typically start between 7:00 a.m. and 10:00 a.m.
  • No call or overnight shifts
  • Average 15–20 patient visits per day

Core Responsibilities

  • Conduct daily rounds in skilled nursing facilities
  • Assess and manage acute and chronic conditions
  • Prescribe and adjust medications; develop and document treatment plans
  • Coordinate care with nursing staff and interdisciplinary teams
  • Ensure compliance with clinical protocols and regulatory requirements
  • Use company-provided iPad (LTE) for documentation and communication

Qualifications

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • Board certification: Nurse Practitioners (ANCC, AANPCB, PNCB, or AACN) or Physician Assistants (NCCPA)
  • Board certified or eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
  • Strong clinical judgment, communication, and independent decision-making skills
  • Ability to collaborate effectively within a multidisciplinary team
