Hospitalist Nurse Practitioner or Physician Assistant — Skilled Nursing/Long-Term Care (IL & Nationwide)

Provide high-quality, patient-centered care to residents in skilled nursing facilities in Cahokia and Hillsboro, IL, with additional openings in MA, MI, NH, NC, OH, and WI.

Location and Schedule

• Full-time positions with flexible scheduling
• Six-hour facility shifts; rounds typically begin between 7:00 a.m. and 10:00 a.m.
• No on-call or overnight responsibilities

Core Responsibilities

• Perform daily rounds in skilled nursing and post-acute care settings
• Manage acute and chronic conditions; prescribe medications and treatment plans
• Document patient encounters using a streamlined EHR with AI scribe and voice dictation
• Use company-provided iPad with LTE and access UpToDate® for clinical guidance
• Collaborate with interdisciplinary care teams to optimize resident outcomes

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• National certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
