Location: Agawam, MA (East and North Agawam) — Full-time
Overview
Provide medical direction and attending physician services to residents in local skilled nursing and long-term care facilities. Work collaboratively with Nurse Practitioners and Physician Assistants to deliver patient-centered, post-acute and long-term care while providing clinical oversight and leadership for assigned facilities.
Core Responsibilities
- Admit, evaluate, manage, and follow patients in skilled nursing facilities
- Provide clinical oversight and guidance to NPs/PAs and facility staff
- Coordinate care plans, transitions, and communication with families and facility teams
- Document care using company-provided iPad with LTE
- Participate in quality initiatives and performance activities as required
Schedule & Clinical Expectations
- Full-time position; typically covers each facility 1–2 days per week
- 18–25 visits per day
- No fixed daily start/end times
- No call responsibilities, no overnight shifts
- Flexible scheduling to support work–life balance
- New graduates who are board-eligible and actively pursuing licensure encouraged to apply

Qualifications
- MD or DO with unrestricted U.S. medical license or eligibility for licensure
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
- Strong clinical decision-making, communication, and leadership skills
- Commitment to compassionate, patient-centered care
