Job Description

Location: Agawam, MA (East and North Agawam) — Full-time

Overview
Provide medical direction and attending physician services to residents in local skilled nursing and long-term care facilities. Work collaboratively with Nurse Practitioners and Physician Assistants to deliver patient-centered, post-acute and long-term care while providing clinical oversight and leadership for assigned facilities.

Core Responsibilities

  • Admit, evaluate, manage, and follow patients in skilled nursing facilities
  • Provide clinical oversight and guidance to NPs/PAs and facility staff
  • Coordinate care plans, transitions, and communication with families and facility teams
  • Document care using company-provided iPad with LTE
  • Participate in quality initiatives and performance activities as required

Schedule & Clinical Expectations

  • Full-time position; typically covers each facility 1–2 days per week
  • 18–25 visits per day
  • No fixed daily start/end times
  • No call responsibilities, no overnight shifts
  • Flexible scheduling to support work–life balance
  • New graduates who are board-eligible and actively pursuing licensure encouraged to apply

Qualifications

  • MD or DO with unrestricted U.S. medical license or eligibility for licensure
  • Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
  • Strong clinical decision-making, communication, and leadership skills
  • Commitment to compassionate, patient-centered care
