Hospitalist Nurse Practitioner / Physician Assistant — Skilled Nursing & Long-Term Care (Charlevoix, MI)

Location: Charlevoix, MI (additional facilities in Carson City, Crystal Falls, East Lansing, Escanaba, Iron River, Mt. Pleasant, South Haven). Additional opportunities nationwide: IL, MI, NH, NC, OH, WI.

Role summary
Provide patient-centered primary and acute care to residents in skilled nursing and post-acute facilities. Work in a collaborative, team-based environment focused on geriatrics and long-term care.

Key responsibilities

• Perform daily rounds (typically start between 7:00–10:00 a.m.) and six-hour facility shifts for full-time providers
• Evaluate and manage acute and chronic conditions
• Prescribe medications and develop treatment plans
• Coordinate with nursing staff, physicians, and interdisciplinary teams
• Document care using company EHR (AI Scribe, voice dictation) on company-provided iPad with LTE
• Use UpToDate® for clinical decision support
• No overnight shifts or on-call duties

Schedule

• Full-time opportunities with flexible scheduling
• Six-hour facility shifts; no call or overnight responsibilities

Qualifications

• Active, unrestricted Nurse Practitioner license
• National certification (ANCC, AANPCB, PNCB, AACN, or equivalent)
• Skilled nursing, long-term care, geriatrics, or post-acute care experience preferred
