Job Description
Provide patient-centered primary care to residents in skilled nursing facilities across Wyoming, Michigan. Part-time role with flexible scheduling, no call or overnight responsibilities. Work collaboratively with physicians, nursing staff, and interdisciplinary teams to manage acute and chronic conditions and ensure regulatory compliance.
Locations
- Primary: Wyoming, MI
- Additional facilities in MI: Bad Axe, Iron River, Mt. Pleasant
- Nationwide opportunities: GA, IL, IN, MO, OH, TN, WI
Schedule and clinical duties
- Part-time; six-hour facility shifts
- Rounds typically start between 7:00 a.m. and 10:00 a.m.
- Average 15–20 patient visits per day
- Daily rounds, assessment and management of acute and chronic conditions, prescribing, treatment-plan development
- Collaborate with facility nursing staff and interdisciplinary teams
- Maintain clinical and regulatory compliance

Requirements
- Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
- NP board certification: ANCC, AANPCB, PNCB, or AACN; PA certification: NCCPA
- Board certified or board eligible in FM, IM, Geriatrics, PM&R, or ED
- Strong clinical judgment, communication, and patient-care skills
- Ability to work independently and collaborate within a multidisciplinary team
