Job Description
We’re expanding our team of clinicians who deliver patient-centered primary care inside skilled nursing facilities across Bad Axe, Michigan. This part-time role offers a flexible schedule, no overnight or on-call duties, and the chance to work closely with physicians and facility staff to provide high-quality, evidence-based care.
Locations
- Primary: Bad Axe, Michigan
- Additional facility options in Michigan: Iron River, Mt. Pleasant, and Wyoming
- Nationwide opportunities also available in GA, IL, IN, MO, OH, TN, and WI
What the role looks like
- Part-time schedule with flexible shift options
- Six-hour facility shifts; rounds usually start between 7:00 a.m. and 10:00 a.m.
- No overnight or on-call responsibilities
- Average of 15–20 patient visits per day
- Conduct daily rounds, manage acute and chronic conditions, prescribe medications, and create treatment plans
- Work collaboratively with nursing staff and interdisciplinary teams to coordinate care
- Maintain clinical and regulatory compliance
- Company provides an iPad with LTE for clinical use

Who we’re looking for
- Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
- Board certification for NPs: ANCC, AANPCB, PNCB, or AACN; for PAs: NCCPA
- Board certified or board eligible in FM, IM, Geriatrics, PM&R, or ED
- Strong clinical judgment, clear communication, and compassionate patient care skills
- Ability to work independently while collaborating effectively with a multidisciplinary team
