Join a growing team dedicated to delivering patient-centered care to residents of skilled nursing facilities across South Haven, Michigan. We’re looking for compassionate Nurse Practitioners and Physician Assistants who enjoy working with older adults in a collaborative, team-based setting and want to make a real difference in patients’ lives.
Where you’ll work
- Primary location: South Haven, MI
- Additional Michigan facility opportunities: Carson City, Charlevoix, Crystal Falls, East Lansing, Escanaba, Iron River, and Mt. Pleasant
- Nationwide openings also available in Illinois, New Hampshire, North Carolina, Ohio, and Wisconsin
What the schedule looks like
- Full-time roles with flexible scheduling
- Typical facility shifts are six hours long; rounds generally start between 7:00 a.m. and 10:00 a.m.
- No overnight shifts and no on-call responsibilities
What you’ll do
- Perform daily rounds and manage both acute and chronic conditions for residents
- Prescribe medications and develop individualized treatment plans
- Document efficiently using a streamlined EHR enhanced with AI Scribe and voice dictation
- Work with a company‑provided iPad (with LTE) and access UpToDate® for real-time clinical support

What we’re looking for
- Active, unrestricted Nurse Practitioner or Physician Assistant license
- Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA)
- Preferred experience in skilled nursing, long-term care, geriatrics, or post-acute care
