Hospitalist Nurse Practitioner or Physician Assistant — Skilled Nursing & Post-Acute Care (Charlevoix, MI)

We are seeking compassionate Nurse Practitioners and Physician Assistants dedicated to improving the health and quality of life of older adults within a collaborative, team-based environment.

Locations

• Primary: Charlevoix, MI
• Additional Michigan sites: Carson City, Crystal Falls, East Lansing, Escanaba, Iron River, Mt. Pleasant, South Haven
• Nationwide opportunities: IL, MI, NH, NC, OH, WI

What you’ll do

• Provide daily rounds in skilled nursing and post-acute settings, managing acute and chronic conditions and developing treatment and medication plans
• Work six-hour facility shifts (rounds typically start between 7:00–10:00 a.m.)
• No on-call or overnight responsibilities
• Document efficiently using a streamlined EHR supported by AI scribe, voice dictation, and a company-provided iPad with LTE
• Use UpToDate® for point-of-care clinical guidance

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• Board certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
