Hospitalist Nurse Practitioner / Physician Assistant — Skilled Nursing & Long-Term Care (Carson City, MI)

Join a growing clinical team delivering patient-centered care to residents in skilled nursing facilities across Carson City, Michigan. We seek compassionate Nurse Practitioners and Physician Assistants dedicated to improving the health and quality of life for older adults in a collaborative, team-based setting.

Locations:

• Primary: Carson City, MI
• Additional Michigan facilities: Charlevoix, Crystal Falls, East Lansing, Escanaba, Iron River, Mt. Pleasant, South Haven
• Nationwide opportunities: IL, NH, NC, OH, WI

What you’ll do:

• Provide daily rounds in skilled nursing and post-acute care settings
• Evaluate and manage acute and chronic conditions, initiate prescriptions and treatment plans
• Work standard facility shifts (six-hour shifts for full-time providers; rounds typically start between 7:00–10:00 a.m.)
• No overnight shifts or on-call requirements
• Document efficiently using a streamlined EHR with AI Scribe, voice dictation, and a company-issued iPad with LTE
• Access UpToDate® for evidence-based clinical support

Qualifications:

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
