Job Description

Provide patient-centered primary care to residents in skilled nursing facilities in Coulterville, Illinois, as part of a collaborative team of physicians and facility staff. This is a part-time hospitalist/general NP role with flexible scheduling and no call or overnight duties.

Locations

  • Primary: Coulterville, IL
  • Additional facilities in IL: Flanagan, Galesburg, Henry, Ridgway
  • Nationwide opportunities: GA, IN, KS, MI, MO, OH, TN, WI

Schedule & Workload

  • Part-time with flexible scheduling
  • Six-hour facility shifts; rounds generally start between 7:00 a.m. and 10:00 a.m.
  • No on-call or overnight responsibilities
  • Average 15–20 patient visits per day

Core Responsibilities

  • Conduct daily rounds in skilled nursing facilities
  • Manage acute and chronic conditions and develop treatment plans
  • Prescribe medications and adjust therapies per evidence-based guidelines
  • Coordinate care with nursing staff and interdisciplinary teams
  • Ensure compliance with clinical and regulatory standards
  • Document care and communicate effectively with facility providers and families

Qualifications

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • NP board certification: ANCC, AANPCB, PNCB, or AACN; PA: NCCPA
  • Board Certified or Board Eligible in FM, IM, Geriatrics, PM&R, or ED
  • Strong clinical judgment, communication, and patient-care skills
  • Ability to work independently and collaboratively within a multidisciplinary team
