Hospitalist Nurse Practitioner / Physician Assistant - Skilled Nursing Facility (Temperance, MI)

We’re expanding a patient-focused team that delivers primary care inside skilled nursing facilities in Temperance, Michigan. This full-time role offers a flexible weekday schedule with no overnight shifts or on-call duties, the chance to work closely with physicians and facility staff, and the ability to provide evidence-based, compassionate care to residents.

What you’ll do

• Work six-hour facility shifts, with rounds generally starting between 7:00 a.m. and 10:00 a.m.
• See roughly 15-20 patients per day, performing daily rounds and coordinating follow-up care
• Evaluate and manage acute and chronic conditions, prescribe medications, and create individualized treatment plans
• Communicate and collaborate with nursing staff and interdisciplinary teams to ensure continuity of care
• Maintain clinical and regulatory compliance and document care appropriately
• Use company-provided technology (iPad with LTE) to support clinical work

Who we’re looking for

• Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
• NP board certification through ANCC, AANPCB, PNCB, or AACN; PAs certified by NCCPA
• Board certified or board eligible in FM, IM, Geriatrics, PM&R, or ED preferred
• Strong clinical judgment, excellent communication skills, and a patient-centered approach
• Comfortable working independently while engaging collaboratively with a multidisciplinary team
