Family Medicine Physician - Team-Based, Value-Based Care in Chula Vista (Managed Panels, Extended Visits)

Join a collaborative, team-based primary care practice in Chula Vista, CA, delivering whole-person, relationship-centered care. We are seeking a Board-Certified Family Medicine physician who is committed to preventive medicine and improving patient outcomes within a value-based care model.

What you’ll do

• Provide comprehensive, preventive, and relationship-based primary care
• Manage a focused, sustainable patient panel (10-15 patients per day) with extended appointment times (20-60 minutes)
• Work closely with an interdisciplinary clinical support team, including health coaches, to coordinate care
• Participate in quality improvement and value-based care initiatives

Qualifications

• MD or DO from an accredited medical school
• Completion of an accredited Family Medicine residency (Family Medicine preferred; Internal Medicine considered)
• Current board certification (ABFM, ABIM, or AOA)
• Minimum of three years primary care experience
• Current, unrestricted state medical license (or ability to obtain)
• Current DEA registration
• Negative TB test within 12 months prior to start date
• Experience in team-based care and familiarity with electronic medical records preferred
• Knowledge of medical coding and documentation, including Medicare Advantage and HCC coding
• Proficiency with Google Workspace
