Outpatient Primary Care Physician (Family Practice) - Upland, CA

Schedule and Setting

• Full-time, Monday-Friday, 8:00 AM-5:00 PM
• Outpatient-only primary care
• Team-based environment with dedicated care coordination and population health resources

Compensation and Benefits

• Base salary: $253,643-$380,464
• Quarterly incentive plan: up to 15% of quarterly earnings
• Long-term incentive: eligible for Profit Shares program
• Bonus opportunities
• Day-one PTO: 160 hours

Qualifications

• MD or DO from an accredited medical school
• Active, unrestricted California medical license
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or related primary care specialty
• Active DEA license
• Commitment to patient-centered, preventive, value-based care
• Strong clinical, communication, and teamwork skills
