Hospitalist Nurse Practitioner / Physician Assistant - Skilled Nursing Facilities (Carson City, MI)

Provide patient-centered primary and acute care to residents in skilled nursing facilities throughout Carson City, Michigan. Work collaboratively with facility staff and interdisciplinary teams to manage chronic and acute conditions, prescribe and adjust medications, and coordinate transitions of care.

Key responsibilities

• Perform daily facility rounds (typically start between 7:00 a.m. and 10:00 a.m.)
• Assess, diagnose, and manage acute and chronic medical issues in older adults
• Develop and implement treatment and medication plans; order and interpret labs and diagnostics
• Coordinate with nursing staff, therapists, families, and external providers
• Complete timely documentation using the company EHR (AI scribe and voice dictation supported)

Schedule and work model

• Full-time opportunities with flexible scheduling

Requirements

• Active, unrestricted NP or PA license
• National certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent)
• Skilled nursing, long-term care, geriatrics, or post-acute care experience preferred
