Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing Care, Lyndhurst OH

Provide high-quality, patient-centered care to residents in skilled nursing and post-acute facilities across Lyndhurst, Ohio as part of a collaborative, team-based practice. You will deliver compassionate management of acute and chronic conditions, develop treatment plans, and prescribe medications while supporting older adults’ health and quality of life.

Key responsibilities

• Conduct daily rounds at facilities (typical start between 7:00-10:00 a.m.).
• Full-time schedules available with six-hour facility shifts; no overnight or on-call duties.
• Assess and manage acute and chronic conditions, coordinate care, and communicate with interdisciplinary teams and families.
• Document efficiently using a streamlined EHR with AI scribe support, voice dictation, and a company-provided iPad with LTE.
• Access UpToDate® for point-of-care clinical guidance.

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA).
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred.
