Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing (Troy, OH)

Join a growing team providing patient-centered care in skilled nursing facilities throughout Troy, Ohio. We are seeking compassionate Nurse Practitioners and Physician Assistants to manage and improve the health of older adults in a collaborative, team-based setting.

Responsibilities

• Full-time role with flexible scheduling; typical facility shifts are six hours and start between 7:00 a.m. and 10:00 a.m.
• Conduct daily rounds, assess and manage acute and chronic conditions, prescribe medications and treatment plans
• Document efficiently using streamlined EHR with AI scribe and voice dictation
• Use company-provided iPad with LTE and access UpToDate® for clinical support
• No call or overnight responsibilities

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• Board certification (ANCC, AANP, PNCB, AACN, or NCCPA)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
