Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing/Post-Acute Care (Archbold, OH)

Provide high-quality, patient-centered care to residents in skilled nursing facilities across Archbold, Ohio as part of a collaborative team serving older adults.

Responsibilities

• Perform daily rounds at assigned facilities (manage acute and chronic conditions, initiate and adjust treatments and medications).
• Complete timely documentation using company EHR (AI scribe, voice dictation available).
• Communicate with interdisciplinary teams, families, and facility staff to coordinate care and transitions.
• Participate in quality improvement and adherence to evidence-based protocols (UpToDate® access provided).
• Support continuity of care and participate in performance and retention programs.

Schedule and Workload

• Full-time positions; typical shifts are six hours per facility.
• Rounds usually start between 7:00 a.m. and 10:00 a.m.
• No overnight shifts and no on-call responsibilities.

Qualifications

• Active, unrestricted NP or PA license.
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA).
