Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing Facilities (Shawneetown, IL)

We are seeking compassionate Nurse Practitioners and Physician Assistants who are committed to improving the health and quality of life of older adults within a collaborative, team-based model.

Key responsibilities

• Provide daily rounds in skilled nursing facilities, managing acute and chronic conditions and developing treatment and medication plans
• Conduct efficient six-hour facility shifts (rounds typically begin between 7:00 a.m. and 10:00 a.m.)
• No on-call or overnight duties
• Document care using a streamlined EHR with AI scribe support and voice dictation
• Utilize company-provided iPad with LTE and access UpToDate® for real-time clinical guidance
• New graduates encouraged to apply

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
