Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing, Battle Creek MI

Join a growing team providing high-quality, patient-centered care to residents in skilled nursing and post-acute facilities across Battle Creek, MI. We are hiring Nurse Practitioners and Physician Assistants to manage acute and chronic conditions for older adults in a collaborative, team-based environment.

Key responsibilities

• Conduct daily rounds at multiple skilled nursing facilities (typical shift 6 hours; rounds start between 7:00-10:00 a.m.)
• Assess residents, develop and implement treatment plans, prescribe medications, and coordinate care
• Manage acute changes in condition and ongoing chronic disease management
• Document efficiently using a streamlined EHR with AI scribe support, voice dictation, and company-provided iPad with LTE
• Use UpToDate® for clinical decision support
• No overnight shifts or on-call responsibilities

Schedule

• Full-time positions with flexible scheduling
• Six-hour facility shifts for full-time providers

Requirements

• Active, unrestricted NP or PA license
• Board certification (e.g., ANCC, AANPCB, PNCB, AACN, or NCCPA)
• Skilled nursing, long-term care, geriatrics, or post-acute care experience preferred
