Hospitalist Nurse Practitioner / Physician Assistant - Skilled Nursing Facilities (Holland, MI)

We’re expanding our team that provides compassionate, patient-centered care in skilled nursing facilities across Holland, Michigan. If you enjoy working with older adults in a collaborative, team-based setting and want a schedule that supports work-life balance, we’d love to hear from you.

What the role looks like

• Full-time position with flexible scheduling and six-hour facility shifts (rounds typically start between 7:00-10:00 a.m.).
• No call or overnight responsibilities.
• You’ll perform daily rounds, manage acute and chronic conditions, prescribe medications and treatment plans, and coordinate care with facility staff and families.
• Documentation is efficient - our streamlined EHR includes AI Scribe, voice dictation, and we provide an iPad with LTE.
• You’ll also have access to UpToDate® for on-the-spot clinical guidance.

Who we’re looking for

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA).
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred.
