Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing, Plainwell MI

Join a growing team delivering high-quality, patient-centered care to residents of skilled nursing facilities in Plainwell, Michigan. We seek compassionate Nurse Practitioners and Physician Assistants who are dedicated to improving the health and quality of life of older adults within a collaborative, team-based setting.

What you’ll do

• Provide daily on-site rounds in skilled nursing facilities, managing acute and chronic conditions and developing treatment plans and medication management.
• Work six-hour facility shifts (rounds typically begin between 7:00 a.m. and 10:00 a.m.).
• No overnight shifts or on-call responsibilities.
• Chart efficiently using a streamlined EHR supported by AI scribe, voice dictation, and a company-provided iPad with LTE.
• Access UpToDate® for real-time clinical guidance.

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA).
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred.
