Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing, Fremont MI

Join a growing team delivering patient-centered care to residents of skilled nursing and post-acute facilities in Fremont, Michigan. Seeking Nurse Practitioners and Physician Assistants committed to high-quality care for older adults in a collaborative, team-based setting.

Key responsibilities

• Perform daily facility rounds (acute and chronic condition management)
• Evaluate patients, develop and implement treatment plans, prescribe medications
• Coordinate with facility staff and families for transitions of care
• Document efficiently using company EHR with AI scribe and voice dictation
• Use provided iPad (LTE) and clinical resources (UpToDate®) for point-of-care decision support

Schedule

• Full-time and PRN opportunities
• Full-time shifts are six hours; rounds typically start between 7:00-10:00 a.m.
• No call and no overnight responsibilities

Requirements

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
