Required Qualifications:

• Current MA LPN license (must be active at profile upload)
• BLS certification
• Valid driver’s license and insurance; ability to drive in inclement weather
• Minimum 1 year acute nursing care experience (hospital-based preferred)
• Home health experience with Certified Home Health required, including proficiency with OASIS
• Infusion therapy experience
• Working knowledge of home care reimbursement and regulatory guidelines
• Demonstrated ability to function independently with minimal supervision, exercise independent judgment, and apply critical thinking
• Strong interpersonal skills; ability to maintain confidentiality and document/communicate clinical data verbally and in writing
• History of flexibility in assignments, dependability in meeting work schedule, and willingness to be flexible with hours
• Experience with self-scheduling of caseload preferred
• Conforms with BHS Customer Service and Performance Standards and Patient’s Rights Policy

Responsibilities:

• Provide skilled nursing care in the home setting consistent with community health principles and agency policies
• Complete and maintain accurate clinical records and OASIS documentation
• Communicate clinical status, changes, and care plans to the interdisciplinary team
• Adhere to all regulatory, reimbursement, and patient rights requirements
• Accept varied assignments and occasional on-call duties as needed
