Hospitalist Nurse Practitioner or Physician Assistant - SNF Primary Care, Temperance MI

Join a growing team dedicated to delivering patient-centered primary care in skilled nursing facilities across Temperance, Michigan. This full-time role offers a flexible schedule with no call or overnight responsibilities and lets you work closely with physicians and facility staff to provide high-quality, evidence-based care.

What the day looks like

• Six-hour facility shifts, with rounds typically starting between 7:00 a.m. and 10:00 a.m.
• No on-call or overnight shifts
• Average caseload of 15-20 patient visits per day
• Daily rounding, management of acute and chronic conditions, medication prescribing, and individualized treatment planning
• Regular collaboration with nursing staff and interdisciplinary teams to coordinate care
• Responsibility for maintaining clinical and regulatory compliance
• Company-provided iPad with LTE for documentation and communication

Who we’re looking for

• Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
• Nurse Practitioners: certification through ANCC, AANPCB, PNCB, or AACN; Physician Assistants: NCCPA certification
• Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
• Strong clinical judgment, excellent communication skills, and a patient-centered approach
• Ability to work independently while partnering effectively with a multidisciplinary team
