We seek a Board Eligible or Board Certified Family Medicine or Internal Medicine physician to join a collaborative team focused on preventive care, chronic disease management, and coordinated treatment.

Location

• Green Valley, Arizona (outpatient primary care)

Team

• Collaborate with Nurse Practitioners, Care Managers, Pharmacists, Social Workers, and clinical support staff.

Key responsibilities

• Provide comprehensive primary care for adult and senior patients
• Diagnose and manage acute, chronic, and complex medical conditions
• Order and interpret diagnostic tests; coordinate specialty referrals
• Collaborate with interdisciplinary care team to optimize outcomes
• Promote preventive care, wellness, and chronic disease management
• Participate in quality improvement and patient engagement initiatives
• Maintain accurate, timely electronic medical record documentation

Qualifications

• MD or DO from an accredited medical school
• Active Arizona medical license or eligible to obtain one
• Current DEA registration
• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Strong communication and collaborative-care skills
• Commitment to value-based, patient-centered care
